Scope and Summary
of Primary ResponsibilitiesThe Client Care Navigator, Federal and Contracted Services (CCN) serves as the primary point of contact for all clients in the Veterans Affairs Canada (VAC) and Non-Insured Health Benefits (NIHB) Programs, as well as other contracted services. The CCN is responsible for completing VAC specific intake and eligibility confirmation, coordinating referrals for VLRC services, monitoring client hours and other related duties. The role will evolve to support services from NIHB, and other contracted service agreements.
The CCN supports clients throughout their care journey by facilitating access to services, coordinating care plans, and strengthening communication between Vision Loss Rehabilitation (VLR) staff and external healthcare providers across the continuum of care.
