Role Description
The Canadian Mental Health Association Sudbury/Manitoulin is seeking a Housing Case Manager for a full-time, contract position. This role focuses on supporting individuals with mental health and substance use issues to enhance their quality of life.
As part of the Community Development & Housing department, you will work closely with clients using a rehabilitation model. Your primary responsibilities will include helping clients secure housing, improving daily living skills, and developing tailored strategies to aid in their well-being. This impactful role makes a substantial difference in the lives of vulnerable populations in Sudbury.
Key Responsibilities:
• Provide comprehensive housing support to at-risk individuals
• Facilitate access to stable housing options
• Enhance client's daily living skills
• Create wellness strategies to maintain living conditions
• Support individuals navigating transitions in justice, homelessness, and healthcare
