This role within our expanding Rehabilitation Team emphasizes improving patient outcomes through thorough assessments and individualized Care Plans. You will work autonomously and collaboratively, leveraging your skills to guide Community Support Workers and ensure effective treatment approaches.
Key Responsibilities:
• Lead supervision and coordination of support staff
• Assess patients in community and home environments
• Develop tailored Care Plans for each patient
• Set and achieve specific, measurable treatment goals
• Ensure compliance with Health and Safety protocols
Requirements:
• Master’s Degree in Occupational Therapy from an accredited institution
• Must be registered in Ontario
• Proven skills in assessments and treatment programming
