Prior nursing experience with a focus on diabetes management is required to excel in this role.
Key Responsibilities:
• Advocate and educate clients about diabetes management
• Plan and facilitate diverse education programs
• Maintain accurate EMR documentation and medical histories
• Collaborate with multidisciplinary healthcare teams
• Conduct outreach events to raise diabetes awareness
