Role Description
Enhance the quality of life for seniors affected by Alzheimer’s and other dementias as a Social Worker with the Alzheimer Society. This full-time, contract position allows for a flexible hybrid work environment.
In this role, you will provide essential support services and education to clients and their care partners. You are expected to be proactive in responding to client needs and facilitating support groups, thus alleviating care partner burdens while promoting access to community resources and services.
Key Responsibilities:
• Timely response to client referrals and needs
• Develop and implement care plans for dementia clients
• Conduct regular follow-ups and supportive counseling as needed
• Facilitate virtual and in-person support groups
• Engage in collaborative efforts with community resource agencies
Requirements:
• Bachelor of Social Work (BSW) or related diploma
• Registered or eligibility for registration with OCSWSSW
