Role Description
Be a vital part of patient care as a Home Health Evening Registered Nurse (RN) with our Agency. You will triage patient calls and provide in-home services Monday through Thursday.
As an Evening RN, your role includes assessing patient needs and coordinating care within a multidisciplinary team. With a focus on quality service, you will execute the nursing process to ensure patient satisfaction. Required skills include effective communication, time management, and the ability to work independently while supporting family needs.
Key Responsibilities:
• Triage patient and family phone calls effectively
• Evaluate and assess patient needs during calls
• Assign and provide quality evening patient care
• Coordinate care with multidisciplinary team members
• Perform intermittent in-home patient care services
