Role Description
Responsibilities
- ED Medical Monitoring focusing on 17‑24 year olds, and monitoring complex adults and youth under 17 as necessary.
- Comprehensive assessment, diagnosis, and treatment planning for acute and complex eating disorder program clients.
- Limited follow‑up care for complex eating disorder program clients.
- Robust discharge planning to reduce readmission rates.
Indirect Patient Care
- MSCM Contract Pre‑approval Routing Form – updated January 2021 3 of 4.
- Liaising between hospital and community program and with Family GP/NP.
- Arranging related assessments, procedures, and therapy; interpreting results.
- Consulting with service providers within the ED program, community partners, and tertiary care.
