Role Description
Contribute to patient care as an Extended Length of Stay (ELOS) Risk Assessor at Health Sciences North, working full-time in a temporary capacity. Focus on screening, patient engagement, and comprehensive healthcare collaboration.
In this role, you will complete risk assessments for extended hospital stays, applying the Home First Philosophy to facilitate timely discharges. You will engage with patients, document findings, and work alongside regulated health professionals to make informed recommendations. This position includes a significant emphasis on teamwork and communication with various healthcare stakeholders.
Key Responsibilities:
• Assist patients with Home First Philosophy assessments
• Complete and analyze screening tools effectively
• Communicate important information across healthcare teams
• Mentor and develop students in the field
• Represent the department in meetings and committees
Requirements:
• Bachelor's Degree in a health-related field
