Role Description
Drive impactful healthcare initiatives as a Team Lead (RN/RPN) for the Homeless Health Program at WellFort Community Health Services. This temporary 8-week contract focuses on leading interprofessional teams effectively.
In your role, you will report directly to the Clinical Community Care Manager and ensure smooth operations within the Homeless Health Program. Your responsibilities will include supervising both in-shelter and outreach teams, along with mentoring staff for professional growth. Engage in planning and conducting team meetings to reinforce best practices and maintain service standards.
