Nurse Practitioner

Bayshore HealthCare is a leading provider of home and community health care services in Canada. We are currently hiring a full‑time Nurse Practitioner to join our Transitional Care Unit in Amherstview, Ontario, supporting seniors transitioning from hospital to community.

Duties and Responsibilities

• Support the needs of patient populations with innovative approaches to care delivery.
• Provide comprehensive primary care as part of an interdisciplinary team focused on creating person‑centered plans of care.
• Review and update primary care activities in line with organizational goals and projected outcomes.
• Promote evidence‑based practice through the use of the latest research‑based guidelines.
