Coleman Care Centre, located in Barrie, Ontario has an exciting opportunity for a REGISTERED DIETITIAN to join their LONG-TERM CARE village team. This is a contract position for a total of 47 hours monthly in Long Term Care.
Responsibilities
- Complete Resident Nutrition Risk Assessments for all new admissions
- Monitor and follow residents who are identified at high nutritional risk
- Determine person responsible for monitoring residents at moderate and low nutritional risk
- Recommend specific nutritional interventions and provide information to the Care Team about implementing recommendations, monitoring and follow‑up as required
- Collaborate with the resident’s attending physician regarding the nutritional plan of care and associated assessment needs, i.e., lab results, vitamin supplementation, diet changes, etc.
