Registered Nurse (Primary Care Navigation Clinic - Integrated Care)

Overview

The Registered Nurse, as part of the Primary Care Navigation Clinic team, works in an interdisciplinary team to deliver high‑quality primary health care services for optimal individual and community health outcomes. The RN coordinates and directs patient care in compliance with the standards of nursing practice and applies clinical and procedural competencies to collect and interpret information, make appropriate clinical decisions, and carry out therapeutic interventions. In collaboration with the interdisciplinary team, the RN applies these competencies within the boundaries of their clinical knowledge, skill, judgment, and the patient’s preferences, guided by codes of ethics and a commitment to clinical competence.

Education

• Registered Nurse with current Certificate of Registration issued by the College of Nurses of Ontario.
• Current BLS required.
• ACLS is an asset.
