Role Description
Support patient health as a Registered Practical Nurse with the Community Health Centres of Northumberland in a part-time capacity. This role focuses on connecting unattached patients to primary care providers in the Northumberland region.
As part of the Supported Attachment Nurse Navigation Program, you will engage in critical patient assessments and navigate care options to ensure patients are well-supported. Collaborating with various primary care practices, you will help foster access to healthcare while addressing barriers to care, ultimately impacting the community’s health equity.
Key Responsibilities:
• Conduct patient health assessments and intakes
• Help new patients through the onboarding process
• Manage documentation and facilitate smooth transitions
• Identify and connect patients to essential health services
• Build relationships with community healthcare partners
