Social Worker (Bethany Residence)

Position Summary:

Working under the direction of the Manager or delegate, this position is responsible for the design, implementation, and delivery of Social Work services across Thrive Group Community Services locations (Community Services). Provides direct counselling, intake and other relevant assessments, advocacy with diverse populations and supports the development and monitoring of client care plans. This position includes supporting clients with a variety of physical and psycho-social conditions inclusive of mental health concerns, system navigation challenges, those marginalized and vulnerable, living in the community and addressing social determinants of health. This position includes delivering client, caregiver and employee education initiatives.
