Position Summary
The Social Worker collaborates with SCPCN internal and external medical home teams to deliver coordinated, high-quality, evidence-based social supports and services. As an integral member of the primary care team, the Social Worker identifies, assesses, and provides best-practice interventions for individuals, families, and groups across all ages and abilities. Responsibilities include assessment, follow-up, consultation, coordination of community resources, and facilitation of psychoeducational workshops.
