Social Worker II

The Social Worker II in the Rockyview General Hospital (RGH) Emergency Department provides essential psychosocial care within a high volume acute care environment. This role supports patients and families presenting with complex social, emotional, safety, and system related needs that impact emergency care, patient flow, and safe discharge. The Social Worker II delivers rapid psychosocial assessment, crisis intervention, brief therapeutic support, and discharge planning for patients across the lifespan. Social work intervention in the ED is critical to addressing social drivers of emergency department use and supporting timely, appropriate transitions to community, home care, or continuing care services. Working collaboratively with physicians, nursing, allied health, EMS, and community partners, the Social Worker II helps mitigate psychosocial risk, inform clinical decision making, and reduce barriers to discharge.
