Reporting administratively to the Manager, Clinical Services and clinically to the Clinical Practice Leader, Social Worker, the Social Worker, Long Term Care uses professional and program standards in providing psycho-social assessment, crisis intervention counselling and specific resident treatment, as well as assisting with tasks that are a cross-responsibility of all team members. Responsibilities Facilitates all admissions to the facility by liaising with Access, Home Health and clients; conducts admission interviews and tours; issues medical forms; completes required documentation; schedules admissions and conducts admission conferences for staff. Assists residents and family members/significant others with financial matters including equipment acquisition, government applications and other matters as appropriate.
