This temporary position with the Ambulatory Program promises to leverage your master’s degree in nursing along with your experience in chronic disease management. You will connect with interdisciplinary teams to ensure seamless care transitions and deliver quality improvements across multiple programs. Collaboration and effective communication with healthcare providers will be key to your success.
