Transitions Caseworker

Job Type: Full-Time ( 8:30-4:30pm, Tues to Sat)

Pay: $24.95 per hour

About the Role

As a Transitions Caseworker, you will play a key front-line role in supporting clients as they navigate pathways out of homelessness and into housing stability. Using a Housing First and Recovery-Oriented approach, you will work alongside individuals to identify barriers and build on personal strengths to promote long‑term, sustainable recovery and independent living. Working as part of our Housing Transitions Team, you’ll support clients through all stages of the housing process—from intake and move‑in to long‑term case planning and community integration.
